 If you feel that there is someone particularly deserving of an Award for their contribution to tennis or their tennis success over the last year then you can make your nomination online by clicking here.

Alternatively you can print a copy off here and return the completed form to Tennis Leicestershire Ltd, Dan Maskell Tennis Centre, Loughborough University, Loughborough LE11 3TU from here. Nominations must be received by 23^rd January 2015.
